General Purpose:
The Registered Nurse (RN) is responsible for providing high -level nursing care requiring specialized knowledge, judgment, and skill derived from the biological and nursing sciences. The RN is the primary professional responsible for assessing health status, developing comprehensive care plans, and supervising the clinical practice of LPNs and STNAs to ensure the safety and dignity of residents in hospitals and long-term care facilities.
Essential Functions:
- Comprehensive Assessment: Performs initial “head-to-toe” and ongoing physical, mental, and functional health assessments to detect changes in resident status.
- Nursing Regimen Management: Identifies nursing diagnoses, establishes a plan of care, and evaluates the resident's response to interventions.
- Medication & IV Therapy: Administers complex medication and treatments, including IV push medications and specialized infusions beyond the LPN scope.
- Clinical Leadership: Delegates during talks to LPNs and STNAs based on an assessment of the resident’s stability and the staff member’s competency.
- Critical Observation: Analyzes lab results and diagnostic tests, reporting significant or abnormal findings to physicians immediately.
- Wound & Infection Control: Leads the facility’s infection control practices and manages complex wound care or surgical site recovery.
- Advocacy & Education: Acts as a liaison between the resident, family, and physician; provides health counseling and explains complex medical plans in plain language.
- Emergency Management: Minimum of one (1) year of professional nursing experience in a clinical setting preferred.
- Certification: Current Basic Life Support (BLS)/CPR certification for healthcare providers.
Knowledge, Skills & Abilities Required:
- Background Check: Must pass mandatory Ohio BCI and FBI fingerprint-based criminal record checks.
- Critical Thinking: Ability to synthesize complex patient data and make independent clinical decisions under pressure.
- Supervision: Skilled in delegating and evaluating the work of other nursing personnel according to the Ohio Nurse Practice Act standards.
- Confidentiality: Strict adherence to HIPAA and patient confidentiality standards, accessing PHI only as necessary for assigned duties.
- Physical Requirements: Ability to stand for long periods and assist in the physical transfer or ambulation of residents.

Licensed Practical Nurse (LPN)

General Purpose:
The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect nursing care to residents in facilities such as nursing homes, hospitals, and long-term care institutions. Working under the direction of a Registered Nurse (RN) or Physician, the LPN executes authorized medical regimens and contributes to the nursing process through data collection and observation.
Essential Functions:
- Medication & Treatment Administration: Administer oral, topical, injectable, and authorized intravenous (IV) medications and treatments according to physician orders and facility policy.
- Observation & Data Collection: Collects and documents objective and subjective data regarding resident health status; reports significant physical or behavioral changes to the RN or Physician
- Clinical Procedures: Performs a wide range of procedures, including wound care, dressing changes, urinary catheterization, and oxygen therapy maintenance
- Supervision: Provides clinical leadership and oversight to State Tested Nursing Assistants (STNAs) to ensure care is delivered safely and effectively.
- Care Planning: Contributes to the development and revision of the nursing plan of care in collaboration with the RN and the interdisciplinary team.
- Health Education: Provides health counseling and teaching to residents and their families regarding medications, treatments, and discharge planning.
- Documentation: Maintains accurate, timely electronic and paper medical records of all care provided and resident response to treatments.
- Emergency Response: Acts quickly in emergencies, performing CPR and basic life support as required.
Minimum Education & Experience Requirements:
- Education: Graduate of an Ohio Board of Nursing (OBN) approved practical nursing program.
- Licensurer: Hold a current, active, and unencumbered LPN license in the State of Ohio.
- Experience: One (1) year of experience in a comparable clinical setting is preferred, particularly in geriatric or long-term care.
- Certification: Current Basic Life Support (BLS)/CPR certification for healthcare providers.
Knowledge, Skills & Abilities Required:
- Background Check: Must pass mandatory Ohio BCI and FBI fingerprint-based criminal record checks.
- Clinical Competency: Demonstrated proficiency in medication safety, physical assessment, and stand nursing procedures.
- Communication: Able to effectively communicate with residents, families, and all levels of the clinical team.
- Technical Skills: Proficiency in using Electronic Health Record (EHR) systems for medical documentation.
- Health Requirements: Meets all applicable health requirements, including negative TB skin test/chest X-ray and required immunizations.

State Tested Nursing Assistant (STNA)

General Purpose:
The STNA and Nurse Aid are responsible for personal care and incidental activities of daily living for patients of all ages in patient facilities such as hospitals, nursing homes, and other institutions. Performs care and delegated tasks and is directed by the nursing plan of care and/or supervisor as permitted by policies and procedures of the facility and allowed under state law/regulation.
Essential Functions:
- Provides for patients' personal hygiene, grooming, and comfort needs, i.e., toileting and elimination, baths, back rubs, shampoos, beds, tubs or showers, skin and nail care, and oral hygiene.
- Promotes a safe environment during care.
- Utilizes infection control measures such as hand washing and the use of personal protective equipment.
- Observes and reports the condition of the patient/client to the appropriate supervisor.
- Works in a team environment to effectively deliver care and assistance to patients and staff.
- Assists with ambulation, transfers, and range of motion exercises.
- Performs patient-related procedures, such as, but not limited to: measuring and documenting intake and output; blood glucose monitoring; urine specimen collections; measurements of vital signs; and weight.
- Administers non-medicated enemas, non-sterile dressings, surgical preps, ice packs, heat treatments, sitz, and therapeutic baths.
- Safely applies and monitors the safe use of physical restraints as directed.
- Transports patients and equipment within the facility as necessary.
- Completes other assignments as requested and assigned.
- May have access to personal health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.
Minimum Education & Experience Requirements:
- Must be 18 years of age
- High School Diploma or equivalent and successful completion of Nursing Assistant course and Nurse Assistant certification as required by the state in which employed.
- A student RN in the last year of school or an unlicensed RN/LPN graduate may be eligible for employment as a Nurse's Aide, as allowable under state law/regulation.
- One (1) year of experience in a comparable job classification and/or as required by a contracted facility.
Knowledge, Skills & Abilities Required:
- Pass criminal and abuse background checks, when required by the State in which they are employed.
- Able to communicate with all levels of clinical and non-clinical individuals.
- Able to read and interpret technical instructions; use vision and hearing to observe the patient.
- Requires hand-eye coordination and fine motor function necessary to assist in the operation of patient care equipment.
- Evidence of current CPR/BLS training for infants, children, and adults.
- Able to provide proof of a valid driver’s license, if applicable.
- Able to provide proof of valid auto liability insurance if assignments include driving one's own vehicle.
- Meets applicable health requirements to provide patient care, as appropriate.
